Banyan Treatment Centers is seeking a dedicated and organized Aftercare Coordinator to support patients as they transition through every stage of their treatment journey. This role focuses on discharge planning, continuity of care, and collaboration with internal teams, families, and external partners to ensure each patient leaves treatment with a clear, well-coordinated aftercare plan. The Aftercare Coordinator plays a critical role in supporting long-term recovery by ensuring no patient falls through the cracks and that all post-treatment needs are addressed with care, accuracy, and compassion.
