Use of office-based pool cars for client home visits About the role We are advertising for a full time Case Manager to join our team in Sydney. Our office is based in Kogarah but we have clients all throughout NSW. Case Management experience is preferred and Diploma of Community Services or Cert IV is essential Your main duties will include: Assessing client needs, developing a collaborative, strengths-based Care Plan with the client and their carer/representative and implementing services which will meet their individual needs
Establishing the Care Plan within an allocated budget, monitoring the budget and ensuring services adhere to the total budget
