Description
Strive Living Society (Strive) is looking for caring and enthusiastic individuals to join our progressive non-profit organization as Support Workers in our Hybrid Supported Home Share program . The Hybrid Supported Home Share Program is a wrap-around model of care that Strive provides to Adults with complex behavioral, social, and medical needs. Individuals are placed into carefully matched family care homes and receive a minimum of 30+ hours of one-to-one support per week, as well as ongoing case management.
