Responsibilities:
- Review and analyze HHC medical records for accurate ICD-10-CM and HCC code assignment
- Ensure documentation supports risk adjustment coding in compliance with CMS and payer guidelines
- Collaborate with clinicians to clarify diagnoses and improve documentation accuracy
- Maintain productivity and quality standards while meeting strict compliance and audit requirements
