Job Description

The System Flow Coordinator is responsible for providing a single point of contact for clients within the Saskatchewan Health Authority (SHA). She or he facilitates physician consults and patient transfers from facility to facility to ensure appropriate patient and system flow across the SHA. The Coordinator collaborates with other stakeholders to complete repatriation of patients back to the appropriate home hospital. She or he coordinates the utilization of the electronic assessment system RAI‑MDS (hereafter referred to as MDS) as the screening tool for Continuing Care eligibility and placement and alternate level of care services placement within the SHA.
