Responsibilities
Collaborative, Community-Based Care
- Deliver comprehensive primary care as part of an interdisciplinary team, ensuring coordinated and person- and family-centred care.
- Collaborate with physicians and allied health professionals to assess, diagnose, treat, and monitor patient health.
- Contribute to continuity of care by connecting patients to programs, services, and community resources.
