Resident Care Planner & Programs Coordinator

THE SUPREME HR ADVISORY PTE. LTD. is looking for a dedicated Social Worker to develop and implement individualised care plans for residents at Sembawang. You will execute care plans, monitor progress, and create programmes that meet the care needs of about 50 residents.

The ideal candidate will have a Degree in Social Work and preferably experience in the Social Service Sector. Working hours are Monday to Thursday from 8am to 5:45pm and Friday from 8am to 4:45pm.
