In this role, you'll focus on guiding families and individuals in navigating the complexities of disability supports. You’ll leverage your knowledge and experience to inform clients about available resources and coordinate meaningful services, ultimately fostering inclusion and independence in the Alberta South region.
Key Responsibilities:
• Offer accurate information on disability supports and services
• Assist clients in accessing and coordinating necessary resources
• Facilitate needs-based assessments and personalized support plans
• Build relationships with community organizations and stakeholders
• Monitor program performance and implement improvements
Requirements:
• Degree in Social Work or equivalent
• 2-3 years of relevant case management experience
