Social Worker – Crisis Resolution and Home Treatment Team (CRHT)

We are looking for a motivated and skilled Social Worker to join our Crisis Resolution and Home Treatment Team (CRHT) in North Lincolnshire. As part of a 24/7 multi-disciplinary service, you will play a key role in providing urgent mental health triage, crisis assessment, and intensive home treatment as an alternative to hospital admission. You will also act as a gatekeeper for inpatient and crisis house admissions, supporting early discharge and ensuring safe, person‑centred care.
Main duties of the job
Working closely with health, social care and community partners, you will use your expertise in the Care Act, Mental Health Act and Mental Capacity Act to complete high‑quality assessments, formulate risk plans, coordinate care packages and promote recovery‑focused, strengths‑based practice.
